Welcome NEW "Groupon" patients!

"Groupon" generates a high volume of calls; our phones may be very busy. Hopefully we can answer some of your questions here, OR leave us a phone message, OR.... send an email. Someone will get back to you before the end of the business day. It's a priviledge to have you as a patient in our office.

Your "Groupon" appointment is with Dr. Tanz for an Examination, Diagnostic X-rays and Regular Teeth Cleaning. Your appointment will take 1 hour. Upon your visit, you will be required, by law, to complete new patient forms including your medical history. We will need your "Groupon" code # in order to schedule your appointment. Our office hours are Monday - Thursday, 9AM to 5PM and we have a strict 48 hour cancellation policy. Our office is on Ventura Blvd. between Hazeltine and Beverly Glen, across the street from the Guitar Center, upstairs on the 2nd floor. There is ample street parking in front.

As a "Groupon" patient you will be entitled to a very special reduced fee schedule for all future dental procedures.
